need to reschedule or cancel?
Your appointment is scheduled in advance. This time is set aside especially for you. If you must reschedule or cancel your appointment, please notify us before 3pm the business day before your appt, otherwise there is a $50 charge. This gives someone else the opportunity to be seen, who may be in pain.
Before Your Visit
1. Complete your patient paperwork in advance
We’ve emailed you a link to set up your profile in our easy-to-use online patient portal. Please create your profile and complete all paperwork on the portal prior to your visit.
If you’re not able to access the portal, click here to download new patient forms. Print, complete, and bring this document packet to your first session.
2. Verify your insurance benefits
In your paperwork, please take care to share your insurance information with us before your appointment.
We offer free insurance verification and with this information, we can best advise you of financial responsibility at your first appointment and you can pay for services at the time of your visit. We do this to help avoid surprises or the need to send you a large bill 3 to 6 months after your care has finished.
Insurance partners we accept
While referrals are necessary for Medicare, no referral is required for most other insurance plans, that’s why we say Try PT First!Aetna, Blue Cross Blue Shield (BCBS), CIGNA HealthCare, Core Source, Liberty Health Share, Humana, Medicare, State Health Plan for Teachers and State Employees, Tricare, Triwest, United Health Care (UHC) … and more
This list includes the major, more common insurance plans. To ensure you’re covered, simply contact us - (919) 410-8840. We will be happy to assist you in billing your insurance company or Medicare.
3. BRing YOUR Referral (If you have one) & List of Prescriptions
While referrals are necessary for Medicare, no referral is required for most other insurance plans, that’s why we say Try PT First! However, if you do have a referral from a doctor or someone on your care team, please bring your referral for physical therapy from your doctor, insurance information, and a list of prescribed medications to your appointment. This way we can update them with your physical therapy progress.
What to Expect & Driving Directions
wear Comfortable clothing & Be ready to stay for about an hour
Come prepared for your initial evaluation with shorts or comfortable clothing. Expect to spend approximately one hour during your first visit. (After your initial visit, you and your therapist will set a schedule which best meets your needs.)

We are conveniently located to visit from anywhere in the Triangle, and we’re very close to Southpoint Mall in Durham, NC - less than a mile north from I-40.
From I-40 West:
EXIT 276 .
Turn right at the exit onto Fayetteville Rd. (away from Southpoint Mall)
Continue on Fayetteville Rd. until you take a left at the 4th stoplight (Geneva Drive, the main entrance to Sutton Station).
Take the 1st left into the parking lot, and we are straight ahead.
